When a tree is cut down, a stump is left behind usually sticking out of the ground anywhere from a few inches high to several inches high.
These stumps have extensive root systems that make them nearly impossible to remove from the ground - the only answer is to use heavy-duty equipment. Depending upon the size of the stump, the grinder sometimes needs to be brought in by truck and hauled on site.
